Lost Mountain Locksmith Service Request Form

Please complete this form in its entirety in order to schedule your appointment with one of Lost Mountain Locksmith's mobile units. Failure to provide all required information will result in a delay in scheduling your appointment.

    Please tell us the type of service you are requesting. If the type of service you select is the "All Keys Lost" option, you must have valid government-issued identification (ex: driver's license, passport, etc.) and proof of ownership (ex: car title, vehicle registration receipt, or bill of sale). We will not accept the vehicle's insurance information as proof of ownership.

  • Vehicle Information

    We make all automotive mechanical keys based on the vehicle having all the original locks installed as it was when it was new from the factory. Please note that if the locks have been changed, the price will be adjusted. If you are unsure if the locks have been changed, that's okay. We will do our best to service the vehicle.

    If the service type requested is for a duplicate key or key fob, please upload a picture of the key. Failure to provide us with a photograph of the key may delay our ability to schedule a service call.
  • Preferred Service Time

    Please indicate the general time frame that you would like to have your service call scheduled. Once we receive your completed form, we will have one of our technicians contact you with a quote for the service requested as well as schedule your service appointment. Our appointments are typically set for two-hour service windows. If this is an emergency, please specify.
  • Although we cannot guarantee that we will be available to schedule your service call on the date selected, we will make every effort to accommodate the date you select.
